The Lifetime Support Authority (LSA) will fund necessary and reasonable equipment to support Lifetime Support Scheme (LSS) participants to complete tasks that are harder, or that they can no longer do, as a result of the motor vehicle injury. The LSA’s funding is subject to the LSS Rules necessary and reasonable criteria and will be referred to when approving funding for equipment requests.
The following documents will assist clinicians prescribing equipment for LSS participants. The forms must be completed and forwarded to the LSS participant Service Planner.
- Prescription Form for Assistive Technology
- Repair Contingency Plan for Assistive Technology
- Medical Information Form: Assessment of Fitness to Drive a Powered Mobility Aid (to be completed prior to all powered mobility aids assessments)
- Participant information sheet – Assistive Technology
- Lifetime Support Scheme Rules
The LSA has in place a preferred panel for complex and/or customised assistive technology.
The benefits of this preferred panel include establishing a supply arrangement that is readily accessible to LSS participants and ensuring service quality through the establishment of suitable terms and conditions with the pre-approved suppliers.
This panel is not exclusive, where there is sufficient justification, complex and customised equipment can be sourced outside of this panel.
The panel is in place until 30 September 2027.
The providers currently on this panel are:
- Active Mobility Pty Ltd
- Aidacare Pty Ltd
- Disability Equipment Services SA
- Homecare Equipment Services
- Kakapo Software trading as All Scooters and Wheelchairs
- Novita Children’s Services
- Fluid Systems Pty Ltd trading as Respirico and Scooterworld
- Xtracare Contracts Pty Ltd trading as Xtracare Equipment
