We will soon introduce updates to several online forms on the LSA website.

What’s changing?

Information Release requests will move to a new online form on the LSA website, replacing the current email submission process.

This change will provide a more consistent and streamlined way for Information Release requests to be submitted and managed.

In addition, the following online forms will be updated:

  • Complaints
  • Feedback
  • Dispute

These updates are intended to:

  • improve the user experience
  • better capture the information required upfront
  • support more efficient and consistent management of requests

What does this mean for you?

There is no immediate action required.

Current forms and email channels will continue to be available until the new forms are introduced.

Once available, the updated forms will provide a clearer and more streamlined way to submit information to LSA through the website. Most forms will remain in similar locations on the website, however some links may change.

These updates are expected to be introduced in July . Please continue to check the LSA website for future updates.
