Working Together to Support Her Journey

Lexi is a bright and determined young girl who continues to thrive through teamwork, understanding , and collaboration.

Lexi lives between her mum and dad’s homes under a shared care arrangement and brings her enthusiasm and love for learning wherever she goes. Born with a vision impairment, she enjoys a range of activities that help build her confidence, coordination, and social connections.

Lexi’s journey is supported through both NDIS and Lifetime Support Scheme (LSS) funding. The collaboration between these two schemes ensures Lexi receives holistic and person-centred care across all areas of her life.

A multi-disciplinary approach involving her Service Planner and key Allied Health Providers plays a key role in keeping everyone informed and working toward the same goals. Regular communication between her planners and therapists helps maintain consistency and understanding across her different environments - from school to home and beyond.

Since starting school, the LSA has helped provide greater clarity around which of Lexi’s needs are related to her motor vehicle accident (MVA). Through LSS funding, Lexi now has access to a comprehensive therapy team including physiotherapy, speech pathology, occupational therapy, and psychology.

The LSA’s approach is holistic and person-centred, ensuring Lexi’s individual circumstances are always considered. Where possible face to face meetings with Lexi’s Service Planner occur before school so Lexi can be part of the conversation and actively contribute to her own planning.

Through the NDIS, Lexi continues to receive support for her social and leisure activities. Early therapy supports helped Lexi establish important foundations for her ongoing development. While the NDIS system can sometimes experience changes in planning personnel, the focus remains on ensuring Lexi’s continued access to meaningful community connections and skill-building opportunities.

Lexi’s resilience began early in life. In December 2016, when she was just four months old, she was struck by a car while being pushed in her pram where she sustained a traumatic brain injury. She required care from the neurosurgical team at the Women’s and Children’s Hospital.

Despite these early challenges, Lexi continues to grow stronger every day. She has been an LSS Participant since 2017, with her interim plan extended until 2029, when her progress and needs will be re-evaluated as she moves into adolescence.

Lexi’s voice is always heard - she takes part in the Participant Forum and enjoys chatting with her planner during visits.

Through strong partnerships and collaboration between Lexi’s family, the LSA, NDIS, and school staff, we’re ensuring that Lexi has every opportunity to continue building her independence, confidence, and love of life.
