If you disagree with a decision about your eligibility for participation in the Lifetime Support Scheme you entitled to lodge a dispute.
Eligibility disputes may be lodged by yourself, a family member or someone else acting on your behalf.
It is very important that eligibility disputes are submitted within six months of you receiving our decision, and must be submitted to us in writing through a letter or email.
Your eligibility dispute letter or email should include:
- name, address and contact details;
- a clear statement that you disagree with our decision;
- detailed reasons why you disagree with our decision; and
- any other information or reports that are relevant to your dispute.
